The role of adjuvant and neoadjuvant therapy in gastric cancer is not fully defined. Neoadjuvant therapy may provide an opportunity to increase the rate of surgical resection of gastric cancer and to provide better results with local therapy. A study of patients with advanced gastric cancer who are candidates for radical resection, in which half receive preoperative chemotherapy and half do not, is being carried out at the University of Colorado. The gastric cancer community is enthusiastic about neoadjuvant therapy, but the facts as we know them now are insufficient to recommend routine use of the technique in the treatment of gastric cancer.
